Back pain physiotherapy for common presentations
- Acute back pain after lifting, twisting or a specific incident
- Chronic lower back pain (more than 3 months) that flares with activity
- Sciatica-style symptoms — pain, tingling or weakness radiating into the leg
- Work-related back pain from sustained postures, manual handling or repetitive tasks
- Post-disc-injury rehabilitation
- Post-surgical recovery after lumbar procedures (decompression, fusion, microdiscectomy)
- Pregnancy and postnatal back pain
- Back pain in older adults related to arthritis or stenosis
- Back pain in athletes — particularly those involved in rotation-heavy or impact sports
What your first back pain appointment may involve
- History — when it started, what makes it better or worse, your work and activity, prior episodes, relevant medical history.
- Physical examination — movement, strength, neurological screening where relevant, specific tests for the suspected source of pain.
- Discussion of what's likely going on — and what's not. We talk through "red flag" features (numbness, bladder/bowel changes, severe night pain, recent trauma) that would warrant medical review.
- Goals and plan — what you're trying to get back to, the proposed plan and frequency.
- Treatment where appropriate — manual therapy, exercise prescription, dry needling where suitable, education on managing the condition.
- Home advice — specific things to try, activity modification, posture and movement strategies.
Back pain physiotherapy treatment may include
- Manual therapy — soft-tissue and joint techniques where clinically appropriate
- Individualised exercise prescription — typically progressed across sessions
- Dry needling where suitable
- Education about your specific back pain, including advice consistent with public resources such as healthdirect Australia's back pain guidance
- Hydrotherapy in selected cases (see our hydrotherapy program)
- Coordination with your GP, surgeon or pain specialist where relevant
What we work on depends on the assessment. Some back pain responds quickly to relatively few sessions; chronic presentations often need a longer plan with active rehabilitation.

Back pain physiotherapy FAQs
Do I need an MRI before seeing a physiotherapist for back pain?
Usually not — for most back pain, imaging doesn't change the early management. Imaging is appropriate when "red flag" features are present or when conservative management hasn't worked over a reasonable period. We'll discuss whether imaging would change the plan.
Should I rest or stay active?
For most acute back pain, gradual return to normal activity is more helpful than prolonged rest. The exception is severe presentations with red flags, which need medical review. Your physiotherapist will guide what's safe.
How many sessions will I need?
It depends on the type of back pain and how it responds. Acute episodes sometimes resolve in 2–4 sessions; chronic presentations often need a longer plan with progressive rehabilitation. We agree timeframes at the first appointment.
Will treatment include hands-on therapy?
Sometimes — manual therapy is one tool among several. For most back pain, active rehabilitation (exercise) does more of the heavy lifting. We use the combination that fits your presentation.
